If you are a provider who wishes to refer one of your clients for a psychological evaluation at MindSights, please download and complete this Provider Referral Form (click here). It is fillable on your computer. Please email the completed form plus any relevant notes, evaluations, etc, to [email protected] or fax them to 503-764-9646. Thank you very much.

We are currently in-network with Columbia Pacific CCO, HealthShare/CareOregon, and Trillium Community Health Plan. As of January 2026 we are no longer in-network/accepting referrals for PacificSource members.

** If your client’s insurance company is Columbia Pacific CCO or Health Share/CareOregon (HS/CO), they require a pre-authorization for psychological evaluations. If you have already secured an CPCCO/HS/CO authorization, please note that the authorization is already present on your referral. Otherwise, we will request an authorization as part of our process with your client. Once Columbia Pacific CCO or HealthShare/CareOregon authorizes services, we will add your client to our waitlist.
